Diabetes Information Classes, Begins January 26 & continue every Thursday through February 16 from 11:30 - 1 pm @ the Nutrition/WIC/Diabetes Building.  Come & join our group & enjoy a light lunch with the program.

* We will talk about healthy eating, activity, diabetes medicine, monitoring, healthy coping & problem solving as well as topics you wish to discuss.

Think of a family caregiver you know and celebrate this special month by reaching out and offering them a helping hand. Bring them dinner, offer transportation. Do something to help.

CTUIR enrolled Elders it is time to sign up for your Medicare Part D, prescription, beginning October 15, 2011 through December 7, 2011.  For more information & assistance with registration you can call 541.966.9830 or come in and speak with any of the Patient Care Coordinators at the registration kiosks at Yellowhawk or Linda Hettinga, Business Office Manager.

Those CTUIR Elders who were enrolled have received booklets and applications in the mail.  Please bring your information into Yellowhawk for assistance in re-enrolling in the program. 

Why do I need to sign up for Medicare Part D?  When we enroll our CTUIR Elders for Medicare D it helps to save limited CHS funds and allows Yellowhawk to collect revenue so Yellowhawk can provide additional services to our patients & community.

 

The first 50 CTUIR Elders that come into Yellowhawk for assistance with signing up for Medicare Part D will receive a gift bag!  We look forward to seeing you!
