Effective January 1, 2010
Yellowhawk pharmacy will no longer be able to accept
new prescriptions for or continue to refill
medications ordered by non-Yellowhawk prescribers
unless one of our providers has referred the patient
to that provider. Prescription orders currently
held by Yellowhawk pharmacy will be transferred to
an outside pharmacy of the patient’s choice.
THE EXCEPTION TO THIS
POLICY IS DESCRIBED BELOW
Prescriptions from
non-Yellowhawk care providers can be accepted if all
of the following conditions are met:

The patient has
established care with the Yellowhawk Tribal
Health Center (YTHC) and has up-dated required
registration information within the past year.

The patient is
under the primary care and management of a
Yellowhawk care provider.

The prescription
is being ordered by a care provider as a result
of a Yellowhawk referral or is a result of an
Emergency Room or Urgent Care visit.

This includes
patients that have been referred to an
internal medicine provider for primary care.
Also effective January
1, 2010 will be the discontinuation of the OTC
program. Except for head lice treatment, the
pharmacy will no longer dispense OTC medications
upon patient request. The few OTC items on
formulary will be limited to dispensing only upon
order by a Yellowhawk authorized care provider.
Patients requesting OTC medications will be
referred retail outlets for the purchase of these
items.
